Applicant Information Page
The Applicant Information page is not only the page in which you provide raw data about yourself it is also the legal document in which you assure us you are who you say you are, and we are allowed to look over your transcripts and other information.
Even though this page should only take you a couple minutes to complete, it is an extremely important part of the application.
THIS IS THE ONLY PAGE OF THE APPLICATION WE MUST RECIEIVE WITH YOU AND A PARENT OR GUARDIAN'S PHYSICAL SIGNATURE.
While the rest of the application may be done purely electronically, on this page we have to see you and your parent's signatures physically present. We do not currently have the capability of reading e-signatures, so don't ask.
This page MUST either be sent in the mail, faxed or scanned into a computer and emailed. Without you and your legal guardian's signature we cannot go forward with your application.
You could have the rest of your application in the bucket, but without this piece of paper we can do nothing for you. So either get a stamp or find a fax machine or a scanner and get this to us first!
Proofs of Legal Residence
With this page you must also include your two Proofs of Legal Residence (details are on page 8 of the application). These can take the form of any document which proves you live in the state of Mississippi. The simplest way to accomplish this is to make a photocopy of both your parents' drivers licenses (you can even use your drivers' license or permit if you have one), and mail or fax it in with your Applicant Information Page. Other acceptable Proofs of Residence include copies of utility bills, pay stubs, tax forms or basically any official document which has one or both of your parents' names and the address where you live printed on it.
