Below, you will find a link to the 2012 Selection Committee Confirmation Form. For your convenience, we have made the process simpler than it has been in previous years. Now, instead of being required to fax or mail in the forms you can fill it out on your computer and email it straight to us. The whole process takes about five minutes.

3. Fill out the form on your computer and check all appropriate boxes.
4. Save the .pdf with a file name of your choice.
5. At this point you can choose to either print the form out and mail or fax it or simply attach the file to an email and shoot it to Wade at wleonard@msms.k12.ms.us
